Department of Health (DOH) Administrative Order 2010-0034, Revised Implementing Rules and Regulations for Massage Clinics and Sauna Bath Establishments, Section 4 Certificate of Registration, 4.2.1 states, “A massage therapist shall apply for the renewal of the Certificate of Registration (COR) and miscellaneous fees for three (3) years. He/she will do this on his or her birth month to any Regional Health Office.
The following are the requirements for the renewal of expired DOH Massage License.
- ID picture 1 x 1 ( 2 pcs )
- ID picture 2 x 2 (1 copy)
- Barangay clearance/cedula
- Any government-issued Identification card
- Professional Tax Receipt (PTR)(City/Municipal Treasurer’s Office)
- Photocopy of Certificate of Registration
- Medical certificate
- Certificate of continuing education (30 Credit Units*) issued by DOH-accredited training institution (ATI).
- Accomplished CEMT Form 3 issued by DOH-accredited training institution.
