Department of Health (DOH) Administrative Order 2010-0034, Revised Implementing Rules and Regulations for Massage Clinics and Sauna Bath Establishments, Section 4 Certificate of Registration, 4.2.1 states, “A massage therapist shall apply for the renewal of the Certificate of Registration (COR) and miscellaneous fees for three (3) years. He/she will do this on his or her birth month to any Regional Health Office.

The following are the requirements for the renewal of expired DOH Massage License. 

  1. ID picture 1 x 1 ( 2 pcs )
  2. ID picture 2 x 2 (1 copy)
  3. Barangay clearance/cedula
  4. Any government-issued Identification card
  5. Professional Tax Receipt (PTR)(City/Municipal Treasurer’s Office)
  6. Photocopy of Certificate of Registration
  7. Medical certificate
  8. Certificate of continuing education (30 Credit Units*) issued by DOH-accredited training institution (ATI).   
  9. Accomplished CEMT Form 3 issued by DOH-accredited training institution.
